Project Overview:
This research proposal aims to assess the impact of a timely hospital follow-up intervention focused on ensuring that patients receive a post-discharge visit within seven days, with particular emphasis on its effect on hospital readmissions. The study will examine outcomes within adult primary care, where more than one year of data are available, and explore expansion opportunities across additional service lines, including vascular surgery and heart failure.
Specifically, the project will evaluate how the combination of timely follow-up and advanced practice provider (APP)-led telemedicine appointments influences readmission rates and continuity of care. The intervention emphasizes both timeliness and accessibility, leveraging virtual platforms to close gaps in post-discharge transitions and reduce preventable readmissions.
Methods and Expected Deliverables:
This retrospective project will include process mapping, data collection, and statistical analysis of outcomes related to readmissions and follow-up timing. Students will participate in understanding the workflow, identifying key metrics, and interpreting findings with guidance from a health system leader. IRB approval will be required to obtain and analyze patient-level data.
The project can be conducted remotely and is well-suited for a very motivated medical student interested in publication output. A dedicated student could complete data analysis and prepare a manuscript for submission by the end of the summer, with findings appropriate for presentation at a Vizient or AAMC conference.

Project Overview:
This research proposal aims to assess the impact of a centralized Value-Based Care (VBC) team, which partners with adult primary care clinics within the UF Health system. Specifically, this study will analyze how the VBC team, which provides care through a telemedicine platform, has influenced key areas of healthcare delivery, including transitional care management, annual wellness visits, Hierarchical Condition Category (HCC) capture, and alignment with system-wide health goals.
In addition to evaluating clinical and operational outcomes, this project will include a cost-savings analysis of the VBC team to demonstrate its value and scalability—particularly as more institutions transition to accountable care and shared savings models. The retrospective study will utilize existing data from UF Health’s population health infrastructure, providing an opportunity for meaningful analysis with real-world implications for care delivery and financial performance.
Methods and Deliverables:
Students will participate in process mapping, data collection, and statistical analysis to evaluate the impact of the VBC model on both quality metrics and the cost of care. An IRB submission will be required to access and analyze patient-level data. The project is well-suited for a motivated student interested in adult primary care, value-based medicine, population health, health administration, or quality improvement.
This project can be completed remotely and offers an excellent opportunity to gain hands-on experience in health systems research. A committed student could complete the full project—including data analysis, manuscript development, and abstract preparation—by the end of the summer. Findings will be positioned for submission to a peer-reviewed journal and for presentation at a Vizient or AAMC conference.
